  CASE HISTORY 8               This is an online e log book to discuss our patient de-identified health data shared after taking his / her / guardians signed informed consent. Here we discuss our individual patients problems through series of inputs from available global online community of experts with an aim to solve those patients clinical problem with collective current best evident based input. I have been given this case to solve in an attempt to understand the topic of " patient clinical data analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and come up with diagnosis and treatment plan . CASE HISTORY 7 This is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. Here, we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs. A 45 years old male patient presented to the opd with chief complaints of :  fever with chills since 4 days  HISTORY OF PRESENT ILLNESS:          patient is apparently well 4 days back with his daily work like farming and then he developed fever with chills where he cannot do his work he went the near by hospital and given medication subsided temporarily and suggested to a big hospital for further treatment . He also complains of generalised body pains and headache  PAST HISTORY  :           patient has no similar history in the past . And No history of diabetes No history of hypertens

A fifteen year old girl presented to OPD with chief complaint of :    Bilateral pedal edema since 15 days    Reduced urine output since 15 days   Abdominal distension since 5 days  Facial puffiness since 3 days  Non productive cough since 3 days  HISTORY OF PRESENT ILLNESS  :                    Patient is apparently asymptomatic 15 days back  And then developed bilateral pedal edema with decreased urine output since 15 days  And also facial puffiness and cough since 3 days Patient developed dypsnea on rest from 3 days . A 45 year old female patient  presented to OPD with a chief complaint of  :   1. Fever since 5 days .  2. Vomiting and headache since 4 days .  3. Shortness of breath since 2 days . HISTORY OF PRESENT ILLNESS :           Patient was apparently asymptomatic 5 days back . And then developed fever which is sudden in onset and gradually increasing .  Fever is not associated with chills and sweating . Fever associated is on and off since 1 month . History of vomiting since 4 days in which food particles are present as content .  History of shortness of breath since 2 days which is more during walking or doing som
